Practice Model & Trauma Informed Care
- Employee Assistance Program
ESSENTIAL FUNCTIONS:
- Conducts mental health assessments and ongoing
risk and safety plans
- Provides ongoing evidence-based clinical
individual and group therapy
- Adhere to all code of ethics, mandated reporting,
and ensure confidentiality of client information including significant
incident reports, CCL, and ORR.
- Inform leadership and federal field specialists
of any confidentiality breaches or third-party request to release
information including those from client's legal counsel
- Screens for human trafficking concerns as well as
other risk and safety issues that may require crisis intervention, safety
planning and/or mandated reporting
- Conduct sponsor contacts to provide psycho
education, safety planning, and family sessions as needed
- Involved in social skills training for each
client
- Meet regularly with cottage/unit staff at staff
meetings, unit meetings and informally
- Take part in selective client activities in the
cottage/unit and maintain awareness of group interaction in the unit
- Coordinate with other staff who provide
case-management and linkage services to ensure alignment across all
services
- Evaluate progress and make recommendations as
necessary
- Serve on agency committees and participate in
staff meetings
- Maintain and complete all required case records
and forms
- Write progress notes in a timely manner and
regularly meet productivity standard for billing
- Other duties as
required
REQUIREMENTS:
- Bilingual in
Spanish Required.
- Master's degree in clinical social work, psychology, sociology, or
other relevant behavioral science in which clinical experience is a
program requirement
- License, waiver and/or registration with California Board of
Behavioral Sciences
- Valid California drivers license, driving record and personal
vehicle insurance acceptable to Hillsides' insurance carrier
- Be able to show proof of immunization or immunity
to vaccine-preventable diseases transmitted by the respiratory route,
including Varicella, MMR, Tdap, & Influenza. Reasonable
accommodations to vaccination requirements available to employees who,
because of a sincerely held religious belief, a medical contraindication,
or a disability, do not receive some or all of the required vaccinations.

INFORMED CARE (TIC)
Hillsides practices Trauma Informed Care (TIC) principles which recognizes the
signs, symptoms, and widespread impact of trauma; Employees are asked to
comprehensively integrate knowledge of Trauma Informed Care into policies,
procedures, and practices with the goal to prevent re-traumatization which
promotes healing of families, youth, individuals, and communities.
