Job Type: Full-time, Non-exempt Opportunity, Monday-Thursday 8:30am-7:00pm.
Starting Salary Range: $18.64 - $22.00 per hour (based on bilingual status and/or experience

ESSENTIAL FUNCTIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.  Other duties may be assigned. The essential functions include:

  • Conduct street outreach to transition-aged youth in a team approach
  • Provide case management, and individual rehab services which include providing information and referrals for social service and financial assistance programs, community resources, recovery supports and treatment providers
  • Maintain and complete record-keeping associated with delivery of services including entering notes and required forms into Clarity HMIS (LAHSA's record keeping system)
  • Facilitate independent living skills workshops, events, and activities
  • Attend and participate in YMO meetings, including staff in-service, clinical conferences, and training retreats
  • Distribute basic needs to YMO program participants
  • Conduct Next Step Tool (LAHSA) homeless assessments to assist TAY youth with potential housing opportunities and provide follow up support including case management and field-based services.
  • Attend CES meetings to provide updates on our Youth Homelessness Demonstration Project (YHDP) contract and update YMO in staff meetings as to new housing opportunities available for TAY.
  • Create partnerships with other service providers in Service Area 3
  • Co-locate some days per week at other SPA 3 drop-in centers to provide outreach and conduct Next Step Tool Assessments.
  • Able to drive for prolonged periods, stretch, reach, push, pull, bend, carry, lift, run, and walk
  • Other duties as required

EDUCATION, EXPERIENCE & CERTIFICATES

  • A minimum of 2 years of related experience working with transitional age youth
  • High School Diploma/GED required
  • Demonstrated ability to work with a culturally diverse clientele & staff
  • Knowledge of prevailing governmental laws and regulations regarding the provision of mental health services, including confidentiality and limits thereof, the reporting of child abuse/neglect, and related regulations
  • Knowledge of Ethical and Professional Standards established by appropriate professional organizations whether or not the individual is a member of any such organization
  • Valid California drivers license, driving record and personal vehicle insurance acceptable to Hillsides' insurance carrier
  • Must have demonstrated the ability to establish and maintain effective person- to person relationships, skill in oral and written communication, ability to inspire confidence and motivate individuals, and capacity for leadership.
